High Street Practice Closure
Sadly our neighbouring practice, High Street has been earmarked for closure. Patients are welcome to join our practice, and have been doing so since the news of the closure was announced. We are delighted to offer High Street patients and all new patients, an optional check-up appointment with one of our nursing staff. We will also ensure all records are transferred from High Street to us so that we have the full medical history and list of any medications.
To join fill in the attached form for each member of your family that is joining and hand it into reception.

Our History
Midway Medical & Walk-In Centre opened its doors to patients on Thursday 2nd April 2009 and we were delighted to be joined by Tony Pulis, Manager from Stoke City.
